Referrals to Heart to Heart Hospice may be made by anyone, including patients, family members, friends, neighbors, physicians, and/or healthcare professionals. Healthcare professionals include: hospital staff, Veteran's Administration staff, Nursing home staff, Home Healthstaff and Assisted Living staff.
Referrals come from those who feel hospice is the most appropriate path for a terminally ill individual, who is seeking comfort, pain management, and symptom management. Heart to Heart Hospice believes hospice is about "Living". Every day is a "gift" to be embraced.
