Patient Care Fund (Non-Profit) |

Heart to Heart Hospice established a Patient Care Fund to help meet the diverse needs of our patients when possible. Medicare, Medicaid and private insurance alone cannot meet every need that may arise due to a terminal illness.
The Patient Care Fund assists with such needs as emergency groceries, assistance with heating/cooling bills, handicap ramps, small home improvements such as grab bars, final wishes (when possible) and assistance with emergency medicines that private insurance does not cover. The fund is also used to mark milestones in the lives of our patients. The Patient Care Fund is a non-profit fund that is supported by donations and fundraisers only.
While it is not possible to meet every final wish, the Patient Care Fund was able to assist with getting a family home for the holidays. It had been a hard year for this family and the patient's health was steadily declining. When asked what the patient would like for Christmas, he replied “ to have my kids at home for Christmas, one last time”. After learning the family was unable to come home for Christmas due to an unfortunate situation, plans were put into action to get his family home. Thanks to the Patient Care Fund, and the help of some dedicated volunteers and employees, the patient's wish came true. His family not only came home for Christmas, Santa delivered Christmas dinner and gifts for all of the children in the house. Many Christmas' have come and gone. However, to those who assisted in making this final wish come true, we shall never forget the joy, love and tears that filled the patient's home the day he learned ho would be getting to see his children for Christmas, one last time.
We also utilize the Patient Care Fund for such things as Christmas gifts and birthday parties for our patients. With the help of the Patient Care Fund, we were able to make one young patient's birthday wish come true with a 300 watt stereo system with a 6 CD changer. Volunteers also brought stuffed animals and pictures of the patient's favorite wrestlers making for a wonderful birthday visit.

Living Wills and Advanced Directives |

There are two types of Advanced Directives.
- Living Will: This is a type of advance directive in which you put in writing your wishes about medical treatment and/or end of life issues should you be unable to communicate at the end of life. Your right to accept or refuse treatment is protected by constitutional and common law.
- Medical Power of Attorney. This is another type of advance directive, sometimes called a “Health Care Proxy” or “Appointment of a Health Care Agent” which enables you to appoint someone you trust to make decisions about your medical care if you can not make those decisions for yourself. In many states, the person you appoint through a Medical Power of Attorney is authorized to speak for you anytime you are unable to make your own medical decisions, not only at the end of life.
An advance directive is a way to ensure that your wishes are followed for medical treatment and end of life issues. You may do this in two ways by making your wishes known in written form called a “Living Will”, and/or choosing to appoint someone you trust as your “Medical Power of Attorney” who will follow your directions and/or make decisions in your behalf if you are unable to make known your wishes or unable to make decisions related to your health and health treatments.

Anyone Can Make a Referral |

Referrals to Heart to Heart Hospice may be made by anyone, including patients, family members, friends, neighbors, physicians, and/or healthcare professionals. Healthcare professionals include: hospital staff, Veteran’s Administration staff, Nursing home staff, Home Health staff and Assisted Living staff.
Referrals come from those who feel hospice is the most appropriate path for a terminally ill individual, who is seeking comfort, pain management, and symptom management. Heart to Heart Hospice believes hospice is about “Living”. Every day is a “gift” to be embraced.
