Heart to Heart Hospice established a Patient Care Fund to help meet the diverse needs of our patients when possible. Medicare, Medicaid and private insurance alone cannot meet every need that may arise due to a terminal illness.
The Patient Care Fund assists with such needs as emergency groceries, assistance with heating/cooling bills, handicap ramps, small home improvements such as grab bars, final wishes (when possible) and assistance with emergency medicines that private insurance does not cover. The fund is also used to mark milestones in the lives of our patients. The Patient Care Fund is a non-profit fund that is supported by donations and fundraisers only. While it is not possible to meet every final wish, the Patient Care Fund was able to assist with getting a family home for the holidays. It had been a hard year for this family and the patient's health was steadily declining. When asked what the patient would like for Christmas, he replied “ to have my kids at home for Christmas, one last time”. After learning the family was unable to come home for Christmas due to an unfortunate situation, plans were put into action to get his family home. Thanks to the Patient Care Fund, and the help of some dedicated volunteers and employees, the patient's wish came true. His family not only came home for Christmas, Santa delivered Christmas dinner and gifts for all of the children in the house. Many Christmas' have come and gone. However, to those who assisted in making this final wish come true, we shall never forget the joy, love and tears that filled the patient's home the day he learned ho would be getting to see his children for Christmas, one last time. We also utilize the Patient Care Fund for such things as Christmas gifts and birthday parties for our patients. With the help of the Patient Care Fund, we were able to make one young patient's birthday wish come true with a 300 watt stereo system with a 6 CD changer. Volunteers also brought stuffed animals and pictures of the patient's favorite wrestlers making for a wonderful birthday visit. You can help provide financial support through donations, memorial gifts or participating in special fundraisers throughout the year.
